You shared your feedback, and we listened: this year, we’re gathering survey responses during registration so you can spend the evening enjoying the celebration.

The registration form includes a brief survey about mental and behavioral health care. Please allow about six minutes to complete registration and the survey. The questions you see will depend on your current work and experience; some attendees will complete only the registration information and a qualifying question.

Your firsthand perspectives will help guide CPF’s work to improve mental and behavioral health access through a grant funded by the Tennessee Department of Health. Responses will inform education, resources for rural clinics, and coordination between primary care and specialty services. No familiarity with the project is needed—we want to hear about the needs and challenges you encounter in your work.
